Moor to Sea

South Devon and Torbay CCG localities map

The Moor to Sea locality covers a large area, which is home to about 44,500 registered patients. It stretches from Widecombe in the Moor in the north to Dartmouth and Chillington in the south, and includes a large part of Dartmoor. There are seven GP practices in the area, all of which are member practices of South Devon and Torbay CCG, and there are three community hospitals – at Dartmouth, Totnes, and Ashburton & Buckfastleigh.

There are many remote communities and small villages across the locality, as well as the main market towns. With such a diverse area and some remote populations, there are differing challenges to meeting the health and social care needs. One of the Moor to Sea team’s aims is to make every referral count by ensuring that all referrals to secondary care are considered and appropriate. They will also develop capacity in primary care to improve management of long-term conditions and urgent care, and ensure good access to appropriate care.

In line with all of the CCG’s localities, as a result of feedback received at a number of public community engagement events in the winter of 2013, the Moor to Sea locality team is developing its community services – including GP services, community nursing, physiotherapy, social care services, and services for carers.
