National Self Care week gave us the ideal opportunity of engaging with and supporting the community, by supporting the Torbay Soup event on 17 November 2016. Voluntary sector and community groups were given the opportunity to pitch for financial support to get a scheme/ idea off the ground and help people get more active, healthy or connected?

Our model for self- care describes a system in wh​ich our patients have the skills, confidence and knowledge to be more active in managing their own physical and mental health.

Empowering people to self-care has many benefits for their short and long term health.  This is important since people are living longer.  Helping people to look after their own health, and their family’s health also helps to manage demand on health services.

The local model for care describes strengthening existing partnerships and creating new ones to promote wellbeing and prevention.  Services will be available closer to home and provided by a wider range of organisations.

We are looking to the voluntary sector to lead the way in building community capacity and resilience.  We will connect people.  By encouraging and supporting the formation of strong peer-support networks we want to create vibrant social and community networks for health.  When invited, we will participate in these networks, and the intelligence this gives us will influence the on-going development of care in South Devon and Torbay.  

We are developing a community grant funding scheme to stimulate innovative ideas to improve health and wellbeing in local communities, whether that be getting families to be more active, or encouraging people with chronic health conditions to manage their own health more successfully.

Kate Rudman, Ocean Youth Radio / Sound communities recorded some personal reflections on what self care meant to people and the role it currently plays or could play in their and others lives, so that we can understand what people need to feel motivated to look after their own health and wellbeing.
