In 2013, South Devon and Torbay was selected as one of 14 areas for the Government’s integration Pioneer scheme. The local scheme is called JoinedUp, and health and social care organisations in South Devon and Torbay have worked closely together with the aim of creating a seamless, integrated system of care for our local population. The initiative is supported by community and voluntary sector groups, who have also contributed, especially in supporting initiatives piloted.

A major milestone was reached in October 2015 when hospital (South Devon Healthcare NHS Foundation Trust) and community services (Torbay and Southern Devon Health and Care NHS Trust) were brought together in the integrated care organisation, Torbay and South Devon NHS Foundation Trust. Since then, many of the Pioneer initiatives have been absorbed into ‘business as usual’.

The overall aim is to join up the health and care system so that patients and people using services don’t have to struggle to get what they need. They will be able to tell their story once, and get coordinated care that really meets their individual needs, and which they will be in control of.
